UK Green Paper on Children and Young People’s Mental Health Provision

UK Government Green paper Transforming Children and Young People’s Mental Health Provision ignores the impact of childhood adversity and social deprivation​

On December 4th 2017 the UK Government released a Green paper entitled Transforming Children and Young People’s Mental Health Provision. The paper was jointly submitted by Jeremy Hunt, now the Secretary of State for Health and Social Care, and Justine Greening, the former Secretary of State for Education who was recently replaced by Damian Hinds.

The government carried out a public consultation asking for people’s views on the paper which closed on March 2nd 2018. We urged everyone to respond:

We wrote the below open letter to Jeremy Hunt and Damian Hinds highlighting our concerns.

RE: UK Government Green Paper, Transforming Children and Young People’s Mental Health Provision, ignores the impact of childhood adversity and social deprivation

No one is in disagreement that children, young people and their families are waiting too long for mental health support and we welcome all action on this issue. We also welcome the commitment by the government to make NHS mental health services better, quicker to access and the overall philosophy of valuing mental health equally to physical health. The government are right to be concerned that children and young people receive the right support at the right time in their lives. It is important too that mental health service provision is prudent and preventative. We also support more access to mental health provision in schools for those who need it.

However, we believe this Green Paper is a huge diversion away from the real issues surrounding children, young people and families’ mental health and, therefore, fails to address the underlying causes of many of these problems. Rising income inequality, poverty, reducing social mobility and the growing number of people living in insecure, overcrowded and unsafe housing are all social issues associated with poorer mental health [1,2,3,4]. Recent research highlights that overall 14 million people now live in poverty in the United Kingdom, over one in five of the population. This is made up of eight million working-age adults and four million children. Eight million people in poverty live in families where at least one person is in work [5].

An increasing body of research identifies the long-term harms that can result from the chronic stress of such socially adverse experiences on children and young people. Evidence shows that these experiences lead to lifelong mental and physical health problems and inter-generational cycles of distress.[6,7] Such rising social deprivation is a consequence of the government’s economic and social policy choices.[8] The implementation of the government’s so-called ‘austerity’ policies over the past six years have served to increase social adversity in communities across the United Kingdom with disastrous knock-on effects for the mental health of the United Kingdom’s most disadvantaged families. These families are now facing the most severe levels of increased social adversity which is contributing to increased levels of psychological distress and mental health diagnoses in children and young people. The Green Paper fails to recognise these impacts and does not address the government’s responsibility in forming policies that will serve to counter this upward trend in young people’s mental health problems.[9] The Green Paper therefore fails in its stated aim to focus on ‘earlier intervention and prevention’ in young people’s mental health problems, because it ignores the significant social and economic contributors to mental health issues faced by children and young people.

Moreover, this Green Paper comes out at a time when the National Union of Teachers has been calling on the government to stop turning schools into ‘exam factories’, which seriously affect the mental health and well-being of pupils. They call for a review of school ‘accountability’ measures which lead to repeated and unhelpful testing of pupils, especially impacting the most disadvantaged students.[10] For as long as this government does not agree to this review, it demonstrates a narrow understanding of how educational policies impact the mental health of pupils.

In response to the Green Paper, we urge the UK government to take a genuinely preventative approach to addressing children and young people’s mental health. This includes acknowledging and addressing the wider societal conditions that are contributing to children and young people’s experiences of mental health. This is not possible without bringing an end to the unnecessary austerity policies in order to reverse the rise in childhood poverty, housing insecurity and loss of statutory services for families and young people, in order to prevent adverse childhood experiences.

Furthermore, we urge the government to take seriously the concerns raised by those with direct experience in relation to children and young people’s mental health, including mental health and educational practitioners, family members and young people with personal experience. We were alarmed to see that the Department of Health and Social Care appeared to dismiss the very real concerns raised by colleagues, such as the Association of Child Psychotherapists, about the potential detrimental impact of current green paper proposals.

We support the calls of colleagues from across the sector urging that the Green Paper’s proposals take a broader approach, and accurately reflect the reality and knowledge on the ground. We therefore urge the government to adopt an approach of co-produced service development, such that all those involved in services have meaningful input into the design of mental health services, including young people who use services, parents, schools, youth workers, mental health workers and others involved in the young people’s lives.

In light of the above considerations, we, the undersigned, call on the new Secretary of State for Education, Damian Hinds, and the Secretary of State for Health and Social Care, Jeremy Hunt, to broaden their ambition and take a public health and community psychology perspective on the mental health of children, young people and their families.

Therefore, we recommend the Green paper be amended to ensure:
  1. The evidence base for socially adverse childhood experiences (experiences associated with social disadvantage and poverty) is accurately accounted for in mental health provision. This should include the following:
    1. Any training provided to health, education and social services staff includes the social determinants of mental ill-health and health inequalities.
    2. Health equity impact assessments to be carried out on all social and economic policies to determine their potential consequences on mental health of families, children and young people.[11]
    3. Prioritising more community based interventions for the prevention and early identification of trauma and emotional harm.
  2. All mental health provision should draw on the principle of co-production. Young people who use services, parents, schools, youth workers, health, voluntary sector, mental health practitioners and residential care staff should be involved in the design and delivery of psychological interventions. Most schools already have arrangements like School Councils and these can be built upon in developing psychological service provision. This co-produced service development must include taking seriously the concerns and feedback from mental health workers and other practitioners who work with children and young people regarding the current green paper and any future proposals.
  3. That a review of the current education system’s ‘accountability’ measures and their impact on pupils’ mental health, as recommended by the National Education Union, is carried out.
